Application for Correction of Name in School Records
Model application to correct a wrongly recorded name before board registration is finalised.
An application giving both versions of the name, the documents that prove it and the urgency.
How to write it
- Give both versions in full: the name as it appears in the school records and the name as it should be. A correction application that does not show both is unusable.
- Say precisely where the error appears — the admission register, the certificate, the registration card — and what the correct source document is.
- Name the documents you are attaching. A name correction is always decided on documentary evidence, not on the student's word.
- Explain how the error arose if you can. It is not required, but it makes the request easier to believe.

Sir
With due respect, I beg to state that I am a student of Class X, Section B, of your school, bearing roll number 31. My name has been recorded in the school register and on my registration card as "Md. Rakibul Islam", whereas my correct name, as it appears on my birth certificate and my Junior School Certificate, is "Md. Rakibul Hasan".
The mistake appears to have been made at the time of admission, when the form was filled in by a relative on my behalf. I did not notice it until my registration card was issued last month. I am enclosing photocopies of my birth certificate, my JSC certificate and my registration card for your inspection, and I can produce the originals whenever required. Unless the record is corrected before the board registration is finalised, the error will be carried into my SSC certificate.
I, therefore, pray and hope that you would be kind enough to correct my name in the school records and forward the correction to the board, and oblige thereby.

Both versions of the name in full, exactly where the wrong one appears, which documents carry the correct one, and the fact that you are attaching copies. Without all four the school cannot act, and the application will simply be returned.
Because a name error becomes far harder to fix once it is carried into a board certificate. Pointing out that registration is about to be finalised gives the school a reason to act now rather than later, and it is a legitimate argument rather than pressure.
It is not required but it helps. A short, plausible explanation — a form filled in by a relative at admission — makes the correction look like a genuine error rather than a change of mind, which is what an authority is guarding against.
