Application for Correction of an SSC Certificate

How to write it
- Identify the certificate exactly — which examination, which year, which board, and the registration and roll numbers.
- State the error and the correct entry side by side, then name the document that proves it.
- Ask the school to forward the application to the board, since a board certificate cannot be corrected by the institution alone.
- Offer to surrender the original certificate, which the board will require.

Sir
With due respect, I beg to state that I passed the Secondary School Certificate examination from your school in 2026 under the Rajshahi Education Board, bearing roll number 145236 and registration number 1912045678. My father's name has been printed on the certificate as "Md. Abdul Karim", whereas his correct name, as recorded on his national identity card and on my birth certificate, is "Md. Abdul Kader".
The error appears to have originated in the registration form submitted in Class IX. I am enclosing photocopies of my birth certificate, my father's national identity card, my registration card and the certificate itself. As a certificate issued by the board cannot be corrected by the school, I request that this application be forwarded to the board with the school's recommendation. I am willing to surrender the original certificate and to pay any fee that the board may require for the correction.
I, therefore, pray and hope that you would be kind enough to forward my application to the board for correction of my certificate and oblige thereby.
